Ongoing, Adaptive Support

Our check-in sessions allow us to track progress, navigate challenges, and fine-tune your plan in real time. This is where lasting change takes shape—through accountability and consistent small steps that add up.

At each visit, we use a medical symptom questionnaire (MSQ) to track your progress over time. This structured, self-reported tool helps highlight both improvements and areas that still need support. It allows us to make data-informed decisions, prioritize next steps, and adjust strategies based on your evolving needs.

Targeted Tools for Deeper Insight

When appropriate, we may use functional lab testing and/or targeted nutraceuticals to support your progress. Labs can offer a deeper look at nutrient status, gut health, inflammation, hormone balance, and other underlying factors contributing to symptoms. If we identify areas that need support, I may recommend medical-grade supplements to help correct imbalances and complement your nutrition and lifestyle plan.
Both testing and supplements are always optional and personalized based on your goals, needs, and budget.

What to expect with each visit

  • During this session we will gather important health information by taking a deep dive into your past medical history and review symptom questionnaires/ intake forms completed prior to first appointment. Below is what to expect with this visit:

    • Intake questionnaire to collect important health information

    • Deep dive into past medical history

    • Recommendations for and ordering functional lab testing (if applicable)

    • Discuss health goals to set the stage for personalized plan, discussed in follow-up.

  • 30-minute initial consultation where we discuss past medical history and health goals. Clients will complete an intake questionnaire prior to appointment that will be used to guide the conversation and personalized nutrition and lifestyle plan. In the follow-up visit, you will work with the provider to review this plan and set realistic goals to begin implementing recommendations.

  • During the follow-up session we will review lab results (if applicable) and review your personalized roadmap to help you achieve your health goals. This may include nutrition education, food plan protocols, exercise/movement plan, stress management tools, strategies for improving sleep quality, and more. 

    Additionally, we will collaborate to create SMART Goals (Specific, Measurable, Achievable, Realistic, and Time Oriented). Small, achievable goals are the key to sustainable lifestyle change and can help you break down your overall health objectives into manageable milestones. 

  • During our 45-minute check-in sessions, we will review your weekly medical symptom questionnaires and discuss any challenges or successes encountered in implementing the personalized nutrition and lifestyle strategies. This iterative process allows for real-time adjustments, ensuring that the interventions align with your evolving health goals. 

    Regular check-ins serve as a crucial accountability mechanism, providing the support and guidance needed to stay on track. This ongoing collaboration not only enhances the effectiveness of the intervention but also fosters a supportive environment that empowers you to achieve and sustain your health objectives.

    As goals become habits we will build upon them, prioritizing based on which next-steps work for your lifestyle and routine.

  • At this time I do not accept health insurance. Upon request, I can provide a superbill for you to submit to your health insurance provider for possible reimbursement.

  • Pricing for lab tests vary but can range from $75 to $600 depending on the test ordered. I do not upcharge labs—I pass on the reduced practitioner costs directly to you.

    Lab testing is entirely optional and is not required to work together. I typically recommend focusing first on foundational nutrition and lifestyle changes. If additional insights are needed, we can then explore functional testing to help guide more targeted support. Recommendations are personalized based on your health history, symptoms, budget, and what we discuss during your initial consultation.

  • All sessions are held virtually via a secure telehealth platform, so you can join from anywhere that’s convenient for you.

  • Every person’s journey is unique, and the timeline for results can vary based on your health history, current symptoms, and consistency with implementing recommendations. This is not a quick-fix approach and real, lasting change takes time.
